What it means is that your sample sits in the lower range of a reference population, and that population includes men who have gone on to father children.</span></p>
<h3><b>What Does Each Parameter Actually Mean?</b></h3>
<h4><b>Concentration and Total Count</b></h4>
<p><span style="font-weight: 400;">Concentration tells you how many sperm there are in each millilitre of your ejaculate, while the total count multiplies that by the volume in order to give you the fuller picture. Both of them matter, because a low volume sample which has a high concentration can still have a perfectly normal total count, and the reverse is true as well. Anything below 16 million/mL gets classified as oligospermia, whereas zero sperm in the ejaculate is azoospermia.</span></p>
<h4><b>Motility</b></h4>
<p><span style="font-weight: 400;">Total motility covers any movement at all. Progressive motility, however, is the figure which matters clinically, because it measures the sperm which are actually swimming forwards, and that is what is required if they are going to reach an egg and fertilise it. Poor progressive motility is called asthenospermia and it is one of the most common causes of male factor infertility there is.</span></p>
<h4><b>Morphology</b></h4>
<p><span style="font-weight: 400;">Morphology is the parameter which is misunderstood most of all. The Kruger strict criteria will only classify a sperm as normal if the head and the midpiece and the tail all meet precise dimensional criteria. So a result of 4% normal means that 96% of your sperm are morphologically abnormal, which sounds alarming when you first read it but which sits entirely within the normal reference range. Moreover, isolated low morphology alongside a normal count and normal motility rarely prevents conception.</span></p>
<h4><b>DNA Fragmentation</b></h4>
<p><span style="font-weight: 400;">A standard semen analysis does not include DNA fragmentation at all. A sperm DNA fragmentation index test, or DFI test, is a separate investigation altogether, and it is typically recommended in cases of recurrent miscarriage, of unexplained infertility, or of repeated </span><a href="https://www.embryologist.co.in/best-ivf-clinic-in-noida-delhi-ncr/"><span style="font-weight: 400;">IVF</span></a><span style="font-weight: 400;"> or </span><a href="https://www.embryologist.co.in/best-icsi-clinic-in-noida-delhi-ncr/"><span style="font-weight: 400;">ICSI</span></a><span style="font-weight: 400;"> failure. High DNA fragmentation is able to affect both your fertilisation and your embryo quality even in the cases where the standard parameters are looking completely normal.</span></p>
<h3><b>What Should You Do With an Abnormal Result?</b></h3>

<h4><b>What Is PESA and When Is It Used?</b></h4>
<p><span style="font-weight: 400;">PESA involves a fine needle passed directly into the epididymis under local anaesthesia. The epididymis is the coiled structure behind each testis where sperm mature and are stored. In obstructive azoospermia the sperm accumulate there, because the downstream pathway is blocked. Aspiration typically yields a good number of motile sperm. The procedure takes minutes. There is no incision. Recovery is rapid.</span></p>
<p><span style="font-weight: 400;">PESA is not suitable for non-obstructive azoospermia. The epididymis in these patients does not contain stored sperm. The problem lies upstream, in the testis itself.</span></p>
<h4><b>What Is TESA and When Is It Preferred?</b></h4>
<p><span style="font-weight: 400;">TESA involves a needle passed directly into the testicular tissue. It aspirates a small core of cells containing sperm. We use it when PESA is unsuccessful or technically difficult. We also use it first-line in some cases of obstructive azoospermia where epididymal access is limited. TESA is sometimes attempted in non-obstructive azoospermia. The yield is lower than micro-TESE there. It samples testicular tissue without identifying which specific areas contain active sperm production.</span></p>
<h4><b>What Is Micro-TESE and Why Is It Considered the Gold Standard for NOA?</b></h4>
<p><span style="font-weight: 400;">Micro-TESE involves opening the testis surgically under an operating microscope. The seminiferous tubules are directly visualised. Tubules that look slightly dilated and more opaque are more likely to contain active sperm production. Those are the ones selectively biopsied.</span> <span style="font-weight: 400;">Micro-TESE achieves sperm retrieval in approximately 40 to 60% of men with non-obstructive azoospermia</span><span style="font-weight: 400;"> who would find sperm in none of their ejaculate. It is more invasive than PESA or TESA. It requires general or spinal anaesthesia. <p><span style="font-weight: 400;">Your endometrial thickness gets measured at nearly every monitoring scan of an IVF cycle at any <a href="https://www.embryologist.co.in/best-ivf-clinic-in-noida-delhi-ncr/">best IVF clinic in Noida Delhi NCR</a>, and hardly anybody explains the number to you until the day it becomes a problem. Which is backwards, really. So here is why the lining matters, what counts as adequate, and what we do about it when it falls short.</span></p>
<h3><b>Why Does Endometrial Thickness Affect IVF Success?</b></h3>
<p><span style="font-weight: 400;">The endometrium, meaning the lining of your uterus, remodels itself every cycle to get ready for implantation, and in an IVF cycle we time that remodelling to your embryo&#8217;s development so the lining is receptive on the day of transfer. A lining that is too thin makes implantation less likely, and so does one with an abnormal structure, and that holds even when the embryo is chromosomally normal and beautifully graded. <h3><b>What Is Considered an Adequate Endometrial Thickness for Transfer?</b></h3>
<p><span style="font-weight: 400;">Most fertility specialists want a minimum of 7 to 8 millimetres by the day of transfer, measured on ultrasound across both layers of the endometrium, which is what the trilaminar pattern refers to. Having said that, it was never a hard cutoff and</span> <span style="font-weight: 400;">pregnancies have been reported on thinner linings</span><span style="font-weight: 400;">. What the data does say is that outcomes below 7mm are consistently less favourable, and that below 6mm the probability of implantation drops off significantly. Pattern counts for as much as thickness does, with a trilaminar or triple-line appearance doing better than a homogeneous or echogenic one.</span></p>

<h3><b>What Can Cause a Thin Endometrium?</b></h3>
<h4><b>What Are the Most Common Causes?</b></h4>
<p><span style="font-weight: 400;">Several factors contribute to inadequate endometrial development:</span></p>
<ul>
<li style="font-weight: 400;" aria-level="1"><b>Insufficient oestrogen stimulation</b><span style="font-weight: 400;">. Natural and minimally stimulated cycles especially</span></li>
<li style="font-weight: 400;" aria-level="1"><b>Previous uterine surgery</b><span style="font-weight: 400;">. Dilation and curettage, myomectomy, prior caesarean section scarring</span></li>
<li style="font-weight: 400;" aria-level="1"><b>Asherman&#8217;s syndrome</b><span style="font-weight: 400;">, where intrauterine adhesions stop the lining growing normally</span></li>

</ul>
<h3><b>What Can Be Done if the Lining Is Inadequate?</b></h3>
<h4><b>What Treatment Options Exist?</b></h4>
<p><span style="font-weight: 400;">Management depends on the cause, but options include:</span></p>
<ul>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Extending or adjusting the oestrogen supplementation. Dose, type, delivery route</span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Low-dose aspirin or sildenafil, off-label, to improve uterine blood flow</span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Platelet-rich plasma (PRP) instillation, or granulocyte colony-stimulating factor (G-CSF). Emerging treatments for a recurrent thin lining, promising early data, not standard practice yet</span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Hysteroscopy. It diagnoses and treats Asherman&#8217;s syndrome, polyps, or fibroids affecting the cavity</span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Freezing all the embryos, then transferring in a later cycle with the endometrial preparation controlled far more carefully</span></li>
</ul>
<p><span style="font-weight: 400;">One scan showing a thinner lining than expected is not the end of a cycle, and the clinical team will usually keep monitoring you before they decide anything. That last option on the list is the one that reaches me most often, and I would rather have it than the alternative, because freezing everything and waiting for a better cycle costs you time and it does not cost you embryos.</span></p>

<p><span style="font-weight: 400;">AMH is one of the most requested blood tests in fertility medicine. It is also one of the most misunderstood. Patients at the </span><a href="https://www.embryologist.co.in/best-ivf-clinic-in-noida-delhi-ncr/"><span style="font-weight: 400;">best IVF clinic in Noida Delhi NCR</span></a><span style="font-weight: 400;"> often arrive having looked the number up already. They have usually drawn a conclusion from it. The number needs more context than a reference range chart can give it.</span></p>
<h3><b>What Does AMH Actually Measure?</b></h3>
<p><span style="font-weight: 400;">Anti-Müllerian hormone is made by the small antral follicles in the ovaries. These are the follicles in the earliest stage of development, before one is selected to ovulate each month. The blood level reflects the size of that pool. It is a reasonable proxy for ovarian reserve, which is the quantity of eggs remaining.</span></p>
<h4><b>What AMH Cannot Tell You</b></h4>
<p><span style="font-weight: 400;">AMH is a measure of quantity, not quality. Specifically, it does not tell you:</span></p>

<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">Whether any particular IVF cycle will end in a pregnancy</span></li>
</ul>
<p><span style="font-weight: 400;">Two women may have the same AMH. Their eggs may behave nothing alike. The difference usually sits in their <a href="https://www.embryologist.co.in/blog/examining-the-relationship-between-age-and-fertility/">age</a>.</span> <span style="font-weight: 400;">Age remains the strongest predictor of egg quality</span><span style="font-weight: 400;">. AMH predicts quantity.</span></p>

<p><span style="font-weight: 400;">The bands vary slightly between laboratories. The value is read alongside the antral follicle count on ultrasound. It is read alongside the clinical picture. It is not read in isolation.</span></p>
<h3><b>Can a Low AMH Prevent Pregnancy?</b></h3>
<p><span style="font-weight: 400;">Not on its own. Fewer follicles are likely to respond to stimulation. Fewer eggs are retrieved. Fewer embryos are available afterwards. That is what a low number predicts. It does not mean pregnancy is impossible. Women with very low AMH have achieved pregnancies through IVF, sometimes on a modified protocol, sometimes across more than one cycle. What matters is realistic expectation and a treatment plan calibrated to what the ovaries are likely to produce.</span></p>
<p><span style="font-weight: 400;">A high reading is not an uncomplicated good result either. Very high AMH, above 5.0 ng/mL and particularly above 7.0 ng/mL, is associated with polycystic ovarian morphology. It is associated with a significantly elevated risk of ovarian hyperstimulation syndrome (OHSS) during stimulation. Clinics that recognise this and adjust the protocol are providing better care than clinics that simply maximise the dose.</span></p>
<h3><b>How Often Does AMH Need to Be Tested?</b></h3>
<p><span style="font-weight: 400;">AMH is relatively stable across the menstrual cycle</span><span style="font-weight: 400;">. FSH and oestradiol are not, and must be drawn on specific cycle days. AMH can be tested at any point, which makes it convenient as a screening tool. It declines with age. In women with conditions like endometriosis, and in women who have had ovarian surgery, it may fall faster than expected. Annual monitoring is reasonable for women tracking their fertility who are not yet planning treatment. Retesting every 6 to 12 months is warranted in women with known borderline or low reserve.</span></p>
