A time-lapse incubator is a sealed culture system which has a microscope and a camera built into it, and it photographs the embryos every five to ten minutes continuously, right from fertilisation through to the blastocyst stage. What you end up with is a detailed video of each embryo’s development, rather than the handful of snapshots which get taken whenever the incubator is opened up for a manual check, and that video is a complete visual record across five to six days.
Two things are happening here at the same time. The second is that the system generates morphokinetic data, which means the precise timing of every single cell division event. When does the embryo divide for the first time? How long does it take going from two cells to three? When does it begin forming a blastocyst? These timings are not visible at all in static images, and they carry real information with them.
What Does Embryo Scoring Actually Mean?
How Algorithms Generate an Embryo Score
At an embryo scoring clinic the algorithms, which are either decision-tree based or machine learning based, analyse that morphokinetic data alongside the standard morphology grading in order to assign each embryo a score which predicts how likely it is to implant. The score combines the following:
- The expansion stage of the blastocoel, which is the fluid-filled cavity, on a 1 to 6 scale
- The inner cell mass grade, or ICM grade, which is the cluster of cells that will go on to form the baby
- The trophectoderm grade, or TE grade, which is the outer cell layer that becomes the placenta
- The division timing parameters, and these are specific to whichever algorithm is being used
What the Score Is Used For
These scores are what allow the embryology team to rank your embryos for transfer or for freezing far more systematically than visual grading on its own would let them, and this matters particularly where several of the embryos look similar on a static assessment.
What Does the Evidence Show on Outcomes?
Here is where honest reporting really matters. The largest randomised controlled trial of a deep learning embryo scoring algorithm to date, which was published in Nature Medicine in 2024, compared AI-assisted embryo selection against standard morphology grading done by trained embryologists, and it did this across more than 1,000 patients. The result was that there was no statistically significant improvement in the clinical pregnancy rates. Where the AI did outperform manual assessment was on speed, because embryo evaluation in the AI group ran approximately ten times faster.
